An advocate requests data about their client (the data subject)
2. Procedure
1) The advocate fills in Part A of this form on DOCX.AM and sends the request. 2) The platform sends the data subject a personal link by SMS and/or email. 3) The data subject opens the link, reads Parts A and B, and signs the consent (with a PIN, Face ID or SMS code) or declines it. 4) Only after the consent is signed does the platform make the request and the advocate get access to the data. Without signed consent no request is made. 5) The data subject sees every request about them on the platform and may withdraw consent at any time, which ends the advocate's access.

4. Part B. Data subject's consent
I, ____________________ (first name, last name, patronymic), PSN or passport/ID card number ____________________, under the RA Law on Protection of Personal Data, consent to DOCIX LLC (Yerevan, Moldovakan St. 2/2, apt. 43, the DOCX.AM platform) obtaining, through the EKENG CJSC identification system, from the electronic register of the Road Police service of the RA Police (Ministry of Internal Affairs), the data listed below about me and the vehicle named in Part A, and providing it to the advocate named in Part A solely for the purpose stated there.
5. Data obtained
1) the owner's first name, last name and patronymic; 2) vehicle type, make and model, VIN, registration plate, registration certificate number, year of manufacture, colour, engine size and type; 3) decisions (administrative acts) on administrative offences committed with the vehicle: decision number and date, type, date, time and place of the offence, the legal provision applied, fine amount, payment status, notification address and date of notification (delivery), and the photo of the offence where available.
6. Purposes
a) automatically filling in data in documents needed to give me legal assistance in the case or document named in Part A; b) preparing an administrative complaint or court claim against administrative acts issued against me, and tracking the appeal deadlines. The data is not used for any other purpose.
7. Access term and conditions
The advocate gets access to the data only after I sign this consent. The consent and the advocate's access last until the legal assistance in the case named in Part A ends or I withdraw consent, and for no more than 6 (six) months. Every request and every time the advocate opens the data is logged. The data is stored encrypted and is available only to me and the named advocate.
8. My rights
I understand that: – I may withdraw this consent at any time on DOCX.AM or by a written request to DOCIX LLC, which immediately ends the advocate's access; withdrawal does not affect the lawfulness of processing before it; – I may see the data processed about me and the requests made, and request correction, blocking or destruction of the data; – every request about me is shown on e-citizen.am; – I may complain about the processing of my data to the Personal Data Protection Agency or to a court.
9. Data subject's signature
Consent is given in writing (handwritten signature) or electronically, confirmed on DOCX.AM with a PIN, Face ID or SMS code. For electronic consent the date, time and device details of the confirmation are kept. Name: ____________________ Signature: ____________________ Date: ____ / ____ / 20___
